Submit Insurance & Address Updates

CompuNet files insurance claims to all primary insurance companies as a courtesy to our patients. In addition, we file secondary claims to selected insurance companies. These companies are Anthem, Medical Mutual, Medicare (all plans), Medicaid (all plans), TRICARE, and United Healthcare.

Insurance information changes frequently. As a result, we request that insurance information be provided each time you have laboratory services performed.

You may provide your insurance information to us by clicking on the link below. You will need your CompuNet billing invoice and your insurance card to complete this screen.
